Assumption of risk & release of liability

I understand that use of the Fox & Maple fitness facility, equipment, and sauna involves inherent risks of physical injury. I voluntarily choose to participate and assume all such risks.

I confirm I am physically able to exercise and have no medical condition that would make use of the facility unsafe. I agree to use all equipment as intended and to follow posted rules and staff instructions.

To the fullest extent permitted by law, I release and hold harmless Fox & Maple RV Resort, its owners, staff, and agents from any liability for injury, loss, or damage arising from my use of the facility, except in cases of gross negligence.

I confirm I am 18 or older, or that a parent/guardian is signing on my behalf. This electronic signature has the same legal effect as a handwritten one.
